Waverly Connect provides personalized care management services, family consultations, placement assistance, resources, and support to individuals and families navigating lifecare needs across Philadelphia and the surrounding counties. We’re here to help you with the stress of challenging situations and decisions you and your loved one or client may be facing, including changing health, mobility, nutrition, and more.

Waverly Connect offers a comprehensive approach to care management, focusing on customized solutions for your loved one. We aim to provide peace of mind and enhance the quality of life in every unique situation. We begin by assessing your loved one’s needs in all areas of their life. From this assessment, we create a roadmap to ensure successful management of the next steps together.
Our care management services also include family consultations, which can help resolve internal family conflicts concerning a loved one’s health and safety. Navigating lifecare changes for a loved one can be difficult, especially when several family members are involved and may not agree on what’s best. Waverly Connect can provide an unbiased view, including information and ideas that will ensure everyone works together toward a common goal and always keeps their loved one’s best interests and needs in mind.

What does an assessment include?
Our assessments offer a holistic view of each individual, covering the following areas:
- Activities of daily living (ADLs)
- Medical needs
- Home safety
- Financial needs
- Mental health
The assessment helps us create a personalized care plan that outlines concerns and recommendations — and also includes community resources for your loved one or client. You can think of the care plan as a roadmap that guides you through each next step.
What counties do you serve?
We serve adults over age 18 throughout Philadelphia, Montgomery, Delaware, and Chester counties. Families and individuals needs vary and there may be instances when Waverly Connect can provide support outside of these counties. I live out of state and am having difficulty caring for my loved one — how can you help?
We understand how challenging it can be to find the best resources, care, and support for your loved one, especially when you live far away. Wavery Connect care managers can be your boots on the ground to ensure your loved one’s needs are met and their best interests are prioritized. We are skilled advocates in care management and can do the following:
- Accompany your loved one to medical appointments.
- Arrange for medical supplies.
- Monitor and coordinate home healthcare.
- Keep families informed about their loved one.
My loved one is no longer safe living alone. Can you help me find the right community to meet their needs and interests?
Yes! Our team excels in helping both professionals and families find the right community to meet their needs. This service, called placement assistance, begins with an assessment of the individual’s living situation and covers the following:
- Strengths.
- Challenges.
- Available support from family, friends, and the community.
- Limitations, including medical conditions, emotional concerns, and cognitive functioning.
How are your services paid for?
Our services are not covered under Medicare or most other insurance plans, making our services a private pay cost to our clients. We do not require a deposit, and we bill our clients monthly for services provided in the month prior. Please reach out to learn more about our fees.
Meet Our Waverly Connect Team
Nicole J. Stroman, MSW, PCA, CMC
Executive Director, Waverly Connect
Nicole, a Care Manager Certified (CMC) through the National Academy of Certified Care Managers, has worked in healthcare since 2006. She holds a master’s in social work and is a member of the Aging Life Care Association®.
Nicole brings considerable experience and expertise in overseeing and coordinating care for clients. In her past professional roles as a Personal Care Home Administrator and as a Social Worker in a skilled nursing setting, Nicole provided counseling and guidance to individuals and their families to help them navigate difficult decisions and moments of crisis.
As Executive Director of Waverly Connect, Nicole continues to provide support and guidance to individuals and families, always with a personalized approach to enhancing a client’s quality of life in a way that is specific to each person’s needs. She also provides overall administrative management and oversight of Waverly Connect. Nicole and her dedicated staff are committed to helping clients explore the many options and resources available for a safe and enjoyable life.
Heather Reilly, CMC
Certified Care Manager
Heather is a Master’s Level Licensed Clinical Social Worker and a Certified Care Manager. She has been a member of the Aging Life Care Association® since 2015 and currently serves on the Board for the Mid-Atlantic Chapter. As a social worker, she worked primarily with families and children. She has also done extensive volunteering with older adults.
Heather joined our team in 2014 and is thrilled to have made the transition to care management. As part of the Waverly Connect team, she provides clients facing health and other life challenges with assessment, planning, communication, and support services. Integral to Waverly Connect’s services is tailoring them to clients and their families.
Meredith Feher, N.H.A.
Senior Vice President of Healthcare Services at Waverly Heights
Meredith has served Waverly Heights as Vice President of Healthcare Services since 2014 and also oversees Waverly Connect. Meredith brings to Waverly Heights more than 20 years of healthcare experience, having served in a leadership position at Abramson Center for Jewish Life, in management at various healthcare facilities, and as a mentor in areas of market development, census strategy, and community relations.
She received her B.S. in Special Education from West Chester University. She is licensed in the Commonwealth of Pennsylvania as a Personal Care Administrator and Nursing Home Administrator.
