Care Management Services — How We Can Help

Waverly Connect offers a wide variety of care management services for adults 18 and older who live throughout Philadelphia, Montgomery, Delaware, and Chester Counties. We support families and their loved ones and also professionals and their clients.

Our care managers serve as advocates, helping individuals navigate challenging lifecare changes and decisions. We coordinate personalized care to improve the quality of life of our clients and ensure peace of mind for their families and professional partners.

A Roadmap for Care

We begin by assessing your loved one’s needs in all areas of his or her life, including activities of daily living (ADLs), medical needs, home safety, financial needs, and mental health. From this assessment, we create a roadmap to ensure successful management of the next steps for your loved one’s care.

How We Can Help

Assessments

Our care management services begin with a holistic assessment of all areas of the individual’s life, including activities of daily living (ADLs), medical needs, home safety, financial needs, and mental health.

  • Activities of daily living: examples include housekeeping, grooming, bathing, eating, driving, dressing, and more.
  • Medical needs: past and current medical situations to better understand how to help manage crises, chronic illness, or unresolved medical issues.
  • Home safety: a detailed review of the home to help reduce the risk of falls or injuries and safety hazards.
  • Financial needs: concerns with paying bills and managing current finances.
  • Mental health: review of current mental health status and concerns regarding dementia or other mental health issues.

Care Plans

After completing the assessment, we create a personalized care plan that identifies concerns and provides recommendations. We will also help coordinate community resources or other care options for your loved one or client. This comprehensive plan will assist everyone involved in effectively managing the next steps.

Family Consultations

Caregiving can be a stressful life event for a family, sometimes leading to conflict and making decision-making and communication particularly difficult. Waverly Connect is dedicated to resolving conflicts within families concerning the health and safety needs of a loved one. At Waverly Connect, we prioritize each client’s needs and provide unbiased information and ideas through a family consultation, ensuring everyone works together toward a common goal.

Placement Assistance

When a different living environment is needed, our dedicated care management team can assist families and professional partners in finding the appropriate community to meet the individual’s needs. Care managers will assess your client or loved one’s strengths, challenges, and needs while identifying medical, emotional, or cognitive concerns. Our team always works in the individual’s best interests, and we do not accept any referral fees from the communities we recommend.

One of the most significant challenges families and professional partners encounter is locating available resources and options. We have been supporting our community since 1993 and have a wealth of information and resources to offer. Our compassionate care managers are here to support your immediate and ongoing needs.
