Care Management

Our care management services begin with an assessment of all areas of the individual’s life, including activities of daily living (ADLs), medical needs, home safety, financial needs, and mental health. Then, we develop a customized care plan that outlines concerns and recommendations — and coordinates community resources or other care solutions for your loved one. This unique roadmap will help you successfully manage the next steps.

Family Consultations

Waverly Connect is dedicated to resolving conflicts within families concerning the health and safety needs of a loved one. Challenges related to lifecare can often lead to stress and conflicts within families, making decision-making and communication particularly difficult. At Waverly Connect, we prioritize each client’s needs and provide unbiased information and ideas through a family consultation, ensuring everyone works together toward a common goal.

Placement Assistance 

When a different living environment is needed, our dedicated care management team can assist families in finding the appropriate community to meet their loved one’s needs. Care managers will assess your loved one’s strengths, challenges, and needs while identifying any medical, emotional, or cognitive concerns. Our team always works in the best interests of your loved one, and we do not accept any referral fees from the communities we recommend.

One of the most significant challenges that families encounter is locating available resources and options. We have a wealth of information and resources to offer. We acknowledge that each situation is unique, so we are dedicated to advocating for your loved one to ensure that their needs are not only met but exceeded.

Our care management team serves as an advocate and partner for adults over 18 years old, ensuring their needs are met and a plan is in place for their ongoing care. Our senior clients appreciate working with certified care managers who follow the Standards of Practice as outlined by the Aging Life Care Association™(ALCA). Our Aging Care Life Care Managers®, also referred to as geriatric care managers are skilled advocates with extensive experience in care for older adults. 

According to Cathryn A. Devons, an assistant clinical professor of geriatrics and palliative medicine at Mount Sinai Hospital in New York City, “Geriatric care managers seek to make the process easier by serving as an advocate or counselor — taking the pressure off of family members who often have other commitments, such as parenting and workplace responsibilities.”

A Waverly Connect care manager will accompany your loved one to appointments, arrange for medical supplies, and monitor and coordinate home health staff as needed. Care managers can step in to ensure your loved one is in the best hands. We’re here to make everything easier for you and your loved one.
